Infant Class Paperwork and Forms.

1. Please review, complete, sign, and bring to class the following forms:
   • Enrollment Packet
   • Parent Agreement
2. Certificate of Immunization Status Form: Washington State Law (RCW 28A.210.160) requires that all children have a completed Certificate of Immunization Status on file at the preschool. Print and complete the form.
3. Please review the following documents:
   • Health Rules
   • Emergency Procedures
   • Committee Positions
   • Bylaws – Revised 2/2013

If you do not have access to a printer, you may request hard copies upon arrival and materials will be available for you in class.
